Patient benefits and coverage are verified prior to the start of treatment and individualized payment plans are available to those without coverage. We’ll be glad to work with you to accommodate your individual needs.
We are currently accepting new patients and we accept virtually all major insurance carriers including commercial, government, and workers’ compensation plans. We also participate in many local provider networks. Our administrative staff will be happy to answer any questions you may have and our software support helps up to keep the paperwork pain-free for our valued patients.
Copayments and deductibles are collected at the time of your visit unless other prior arrangements are made. We accept cash, personal checks, and most credit cards. There is a $35.00 returned check fee.
* It is very important that you provide us with the most current insurance information. Failure to provide us with the most current information will result in your health insurance company’s denial to pay for services which will leave you responsible for the payment. If you do not have proof of insurance at the time of your visit you will be expected to pay for the visit at that time. Please note that it will be in your best interest to know and understand your insurance policy, benefits, out-of-pocket cost (deductible and/or copayments), and authorization requirements prior to your first therapy visit. The patient is ultimately responsible for knowing the details of your health plan coverage. We will always encourage you to contact your insurance company so that you know and understand your benefits prior to your initial appointment.
